DV = Stands for the Diversity Visa

PROGRAM = Stands for PROGRAM

STATE = Refers to the Department of State (in charge of the lottery process)

GOV = Refers to the Government

If you go right now on the website, you will not see the application form yet. The DV Lottery Application form will be accessed on the application window dates.
